The ceiling has been painted but within twelve months the paint started to peel off in large pieces. What is the reason for this?

Could be many reasons… improper or no primer, poor quality finish coat paint, improper application… leaking upper area above ceiling (roof?)… or maybe you have a leaking water pipe above…
