The wall was wallpapered and when I removed the paper the wall was discolored in a few places, and now they are showing up on the freshly painted walls which has 3 coats.

Sounds like you have some stains bleeding through the paint. These areas will need some stain blocking primer then touching up. Use either an oil base primer, Zinnser Cover Stain or equivalent, or a shellack based primer. Shellack primers are the best for stains.
